My right testicle has been enlarged for 3 months now. it has slight pain. ive been to the doctor 5 times.?

i had an ultrasound done about a month and a half ago. the doctor said i had a hydrocele, and he didnt see anything that looked like cancer, 2 weeks went by and no relief, i went back to the doctor and said it was varicocele. no relief still went back to the doctor and had bloodwork done and nothing was abnormal. now my right testicle is aching, swollen and a vein has formed on it. does anybody have any idea?
